Grief in the Body: When Loss Lives Beyond Words.

Grief is often associated with bereavement, but from a neuroscience and physiology perspective it is a whole body adaptation to loss, change, and disrupted attachment. In my work, I encounter grief not only after death, but in chronic illness, changes in physical capacity, shifts in identity, relationship endings, and the ongoing experience of life not unfolding as expected.

Neuroscience shows us that grief is deeply linked to the brain’s attachment and threat-regulation systems. When something or someone significant is lost, the limbic system continues to signal “seeking” and “missing,” which is why grief can feel persistent and physical rather than purely emotional. At the same time, the autonomic nervous system shifts between sympathetic activation (restlessness, anxiety, agitation, searching behaviours) and dorsal vagal shutdown (fatigue, numbness, withdrawal). This oscillation is not psychological inconsistency; it is biological regulation attempting to restore safety after disruption.
This is why grief often presents in the body: changes in sleep, appetite, digestion, breath pattern, muscle tone, and energy levels are all mediated by autonomic nervous system activity. Cortisol and adrenaline patterns may become dysregulated in early grief, contributing to hypervigilance or exhaustion, while reduced dopamine activity can affect motivation and pleasure. These responses are not signs of dysfunction but of an organism recalibrating after loss of attachment, routine, or meaning.
One of the most common misunderstandings I see is the belief that grief should follow a predictable timeline, or that ongoing waves of emotion indicate something unresolved or “wrong.” In reality, contemporary models of grief in psychology and neuroscience have moved away from linear stages and instead describe a process of oscillation and integration. The nervous system does not process loss once; it revisits it in cycles as capacity and safety change over time.
This is particularly relevant in chronic illness and long-term health conditions, where grief is often ongoing rather than singular. Conditions such as rheumatoid arthritis, multiple sclerosis, Parkinson’s disease, chronic fatigue syndrome / ME, fibromyalgia, long COVID, and chronic pain conditions all involve repeated micro-losses: of energy, physical ability, independence, and aspects of identity. Each flare-up or functional change can be processed by the nervous system as a form of loss, which is then layered onto previous experiences. Over time, this can create a cumulative load on the autonomic nervous system, where the body may shift between states of persistent activation (anxiety, vigilance, tension) and shutdown (fatigue, withdrawal, low mood). This is not a sign of psychological fragility, but of a system adapting to repeated disruption without full opportunity for recovery or integration.
From a regulatory perspective, the goal is not to eliminate grief, but to support the nervous system’s ability to move flexibly between states. Simple orienting practices, such as visually scanning the environment, noticing sensory input, or bringing attention to breath without force, activate cortical networks that signal present-moment safety, helping to reduce limbic overactivation. Gentle interoceptive awareness, such as feeling the contact of the body with the ground, can support vagal regulation and downshift physiological arousal.
Grief, then, is not just emotional processing. It is a neurobiological reorganisation of safety, attachment, and meaning. It reflects the body’s attempt to adapt to absence while continuing to function in the present.
If there is anything to take from this, it is that grief is not linear, and it is not a sign of failure to still feel it. It is evidence of connection, adaptation, and a nervous system doing exactly what it is designed to do.
